Microsuction vs Irrigation vs Manual Ear Wax Removal: Which Is Right?
Three professional methods remove ear wax: microsuction (a low-pressure vacuum), irrigation (warm water), and manual extraction (curettes and forceps). All three are safe in trained hands. The right one depends on your ear, your wax, and your medical history. Below I'll explain when each is the right choice, and what you should never do at home.
Microsuction
Microsuction uses a fine surgical-grade suction probe to gently vacuum wax out of the ear canal under direct visual magnification (either head loupes or a video otoscope). It's the method of choice in most modern clinics, including mine, and it's what NHS ENT departments typically use.
Why it's the gold standard
Direct vision: I can see what I'm doing throughout
No water involved: safe for perforated eardrums, grommets, mastoid cavities and previous ear surgery
Precise: I can target just the wax without disturbing the canal skin
Quick: usually 5 to 15 minutes per ear
Comfortable for most patients
What it feels like
You hear a vacuum noise. You may feel a light tickling or pulling sensation. There's no pain in normal circumstances. Some patients find the noise startling at first. I always warn before starting.
Irrigation
Irrigation uses a pump that delivers warm water (at body temperature, low pressure) into the ear canal to flush out wax. It's effective for softer wax that's built up in the outer canal, and for the kind of long-standing wax that needs gentle dissolution rather than pinpoint suction.
When I use it
Wax that's very deep and adherent to the canal wall
Wax that's already softened by olive oil drops
Patients with very narrow canals where microsuction tip access is limited
Specific anatomies where suction is less effective
When I avoid it
Any history of eardrum perforation that hasn’t resolved
Grommets in place
Previous ear surgery (mastoid cavity, stapedectomy)
Active ear infection or recent infection
Single hearing ear (the side you rely on). I'd use suction instead.
Dizziness or vertigo conditions
What it feels like
A sensation of warm water in the ear canal and a soft rushing noise. It shouldn't be painful. The water comes out into a basin held under your ear. Some patients feel briefly dizzy as the water's temperature interacts with the inner ear (the caloric effect). It passes in seconds.
Manual extraction
Manual extraction uses tiny curettes, hooks or forceps to physically lift wax out under direct vision. It's the oldest method and still useful.
When I use it
Hard wax fragments sitting near the entrance to the canal
Foreign bodies (cotton bud tips, hearing aid domes, the occasional insect)
Touch-ups during a microsuction session
Most appointments use a combination. I might start with manual extraction for the easily accessible bits, switch to microsuction for the deeper stuff, and finish with a final visual check.
What to avoid at home
I see the results of these every week:
Cotton buds: they push wax deeper, scratch the canal, and cause impactions. Don't.
Ear candles: no evidence they work, and a real risk of burns and wax in the canal from the candle itself. The FDA has issued warnings.
Online ear-cleaning tools (the curette-and-camera devices marketed on social media): easy to injure your eardrum without training. I've removed several broken tips from canals.
Olive oil for weeks at a time: short-term softening is fine, long-term use can cause skin issues and trap debris.
Hydrogen peroxide ear drops: useful in some clinical contexts but can irritate canal skin. Check with a clinician first.
The before and after
At the Hear Now Clinic every patient sees images of their canal before and after the procedure on our video otoscope, with the anatomy pointed out. If anything looks unusual (granulation, fungal infection, suspected eardrum issue) we'll refer onwards.

Wax removal at Hear Now Clinic is £50 for one ear, £70 for two ears (with a complimentary hearing screener). Appointments run 30 minutes. Most are bookable within the same week, often the same day for urgent cases.
If wax is causing pain, sudden hearing loss, dizziness or has been impacted for weeks, don't wait. Get it removed properly.
Frequently asked questions
Should I use olive oil before my appointment? Yes. 3 to 5 drops a night for 3 to 5 nights before makes wax easier to remove. Don't overdo it. A week is plenty.
Does microsuction hurt? Not normally. You'll hear the suction noise and feel a tickling sensation. Pain isn't typical. If something's sore, we stop and look.
How often should I have my ears cleaned? It varies. Some people produce wax fast and need attention every 6 months. Others never need it. If you wear hearing aids, expect 6 to 12 month intervals.
Can I do it myself with a curette I bought online? I'd strongly advise against it. Without magnification and training, the risk of perforation is real. I've removed broken tips from people who tried.
Is wax removal safe with a perforated eardrum? Microsuction and manual extraction are. Irrigation is not. Always tell your clinician about your ear history.
Can I drive after wax removal? Yes. Some patients feel briefly dizzy after irrigation but it settles in seconds. Microsuction has no such effect.
